Record Description : This series contains: Dependent care spending account documents for enrollment and change of status request (ADM 4307), dependent care spending account withdrawl request (ADM 4308) to verify expenses in order to be reimbursed from their spending account. Dependent care spending account participant files wich include application, a copy of the conformation letter, a signature letter from the provider, iff applicable, processed withdrawl requests, and other miscellaneous communication.
